- Mail the following required items to the address below by April 10 (items must be received by this date):
- Transcripts - REQUIRED
- A copy of your community college Educational Plan
- OFFICIAL transcripts from all colleges and/or universities attended (in their original sealed envelope)
- UNOFFICIAL transcript from high school from which you graduated (copies from your community college file are acceptable)
- Letters of Recommendation - REQUIRED
- Two (2) letters of recommendation are required. At least one must come from a community college faculty member.
